I hereby acknowledge that I have voluntarily enrolled my child, in the Open Acro Class conducted by Acrowithbethat In consideration of the opportunity to participate in the Acro class, I agree to the following:

1. Assumption of Risk: I understand and acknowledge that participating in Acro classes involves inherent risks, including but not limited to, the risk of injury, falls, collisions, and accidents. I accept full responsibility for any and all risks, known or unknown, arising from my child's participation in the Acro class.

2. Medical Treatment Authorization: I authorize the Instructor and their representatives to seek and obtain medical treatment for my child in the event of any illness, injury, or medical emergency that may occur during the Acro class. I understand that the Instructor will make reasonable efforts to contact me or the emergency contacts listed above before seeking medical treatment, but I agree that such treatment may be administered without prior consent if deemed necessary by the Instructor.

3 Photo/Video Release: I grant permission for the Instructor to use photographs, videos, or other recordings of my child taken during the Acro class for promotional or educational purposes, including but not limited to, advertising, social media, websites, and publications.
